Set Up Family Account

Fill out the client form to set up your family account to get started!

Pick Up Diapers

Stop by the Main Office at Cooks Hill to pick up your first set of diapers.

You will need to bring with you:

Parent/guardian ID
Proof of Address (Lewis County Residents only)
Proof of birth for children needing diapers (health card, birth certificate, etc.)

2400 Cooks Hill Rd, 9am -12pm, Thursdays

*Visits outside of these hours will be asked to return at a later date.

Set a Reminder!

You can access a refill voucher given out by the office every 30 days for a free 3 day supply of diapers. Refills will not be given out without the refill voucher.
